Once you have completed the fit to fly application form below, I will forward your official Fit to fly note / assessment response to the email you will have supplied on the form .

Most applications are straight forward and I can quickly issue your official document, however in some cases I may need to contact you for further clarification or information, so please ensure your details are correct on the form.

Cost for my assessment is:

£35 regular service (within 1 week of form submission)

£60 express service (if you require the response within 3 days)

Please Note:

This service is for my assessment of your fitness to fly. 

While there are very few conditions where you would not be deemed fit to fly, and the vast majority of applications do indeed result in a successful issue of an official 'Fit to Fly' note, your application is not a guarantee of this, and no refund will be made if I consider that, as a registered midwife, that you are not fit to fly.
